Both signs mean the same, “really” but you will see many Deaf people signing “REAL” and then fingerspell “L-Y” for “really”, as in “really??” and also as in “very”. . This sign, “REAL L-Y” is not in sign language books and most likely not taught in ASL classes either. Signs like these, you learn through socializing with Deaf people, in the Deaf community.
